Please note that if you fail to provide any of the above information then your medical examination cannot be completed and would need to be cancelled. You will incur another charge for any rescheduled appointment booked.
IMPORTANT ADDITIONAL NOTE:
After seeing the doctor please take the time to review your D4 form to ensure all questions have been dealt with. If any have been missed then wait to go back to see the doctor before you leave the clinic so it can be sorted out there and then. Otherwise the DVLA may return your form to you and ask for it to be completed at a later date, which may delay your licence.
Please also note that if the doctor finds that your sight does not meet the criteria for the medical or you fail your eyesight test, then the doctor will give you the option to complete the rest the of the medical examination together with the vision part of the form, or you can visit an optician who should be able to recommend corrective measures so you do meet the criteria required. Any repeat medical examinations as a result will incur another charge.
The Post Office also offer a checking service at an additional charge to check all the information has been provided before sending it to the DVLA, particularly if you need your licence within a certain timeframe.
If you do not check the form before submission to the DVLA and they return the form then you can post it to the doctor direct to have it dealt with and returned to you for resubmission. However, we do not accept any responsibility or any consequential losses whatsoever as a result of it. You accept that the doctor has conducted your medical in their own professional capacity and therefore agree to liaise with them directly without recourse to Us if there are any issues arising from the medical. This is because the DVLA’s literature explains that it is your responsibility to check the form for any missing information before submitting it to the DVLA.
The full fee is payable for the medical examination at the time of booking by debit or credit card. The fee is non-refundable except in the case of an appointment cancelled with more than 5 full working days’ notice before the booked appointment.
If you cannot or prefer not to pay by card, you can contact us by telephone to arrange to send us a postal order and we will contact you to book the appointment once we receive it. Please note there is an additional charge of £3 for this service.
If an existing appointment is cancelled with more than 5 full working days’ notice then a refund will be given. Any rescheduled appointment will need to be paid for in full.
If an existing appointment is cancelled with 5 full working days, or less than 5 full working days’ notice and rearranged at the same time for a different date, a full charge will still be made for the cancelled appointment, but at our discretion and depending on the circumstances a reduction of £18 will be applied to any new appointment so that the second appointment will cost £30. You will need to contact us by telephone to arrange this. However, this does not apply to appointments which are not attended on the day with no prior notice and subsequently rescheduled.
No refund will be given for medicals not attended or which have been cancelled with less than 5 working days’ notice of the booked appointment date.
If We are unable to provide a medical examination which has been booked, Our liability will be limited to refunding any sums paid for the medical if we cannot arrange an alternative date or location. We will not be liable for any consequential losses in the circumstances.
Under the Consumer Contracts Regulations you have the right to cancel a contract within 14 days of the booked appointment date. The contracted service is deemed to begin from the beginning of the day of the booking. However, the right to cancel is lost during this period if the service is provided in full or preparations have already been made in preparation for the appointment.
